Community Impact Grant Agency Report

United Way Community Impact Grants allow for specific projects that will impact the lives of citizens of the Northern Shenandoah Valley in a positive way.  We ask each recipient agency to complete the following report as a record of the outcomes and benefits made possible through United Way grant funds.

Agencies, please submit your quarterly reports using the form below. Please note some questions are required to be answered with the whole grant year in mind and others are specifically for the report period. Please contact the United Way NSV office if you have questions 540-536-1610.

September reporting is for outcomes July 1-September 30, December Reporting is for outcomes October 1-December 31, March reporting is for outcomes January 1-March 31, June reporting is a YEAR end report for the entire grant cycle July 1-June 30.

What was the funding granted to support?
What are the OUTCOME objects of your grant for the entire grant cycle (one year)? What do you hope to achieve?
Inputs are the resources dedicated to the program - how were grant monies spent?
What were the program activities for the report period? Activities are what the program does to fulfill it's mission (eg. feed and shelter homeless, provide job training, etc.)
Outputs are the direct products of program activities (eg. number of clients served, number of classes taught, counseling sessions conducted, etc.). This number should be for the REPORT PERIOD.
What are the outcomes of your grant for THIS report period (3 month period)? How were lives changed in this quarter because of the grant funding received? OUTCOMES are the benefits for participants before and after program activities.
